About the role
At Your Door: Visiting Healthcare Services/National Church Residences – Central Ohio is hiring a Nurse Practitioner to serve Assisted Living, Independent Living, and Skilled Nursing Facilities in the Central Ohio area. This PRN position offers flexible hours, PTO, and the potential to become full-time as the market expands. The role emphasizes autonomy, geriatric care, and a supportive team environment.
Responsibilities
- Conduct physical exams and comprehensive medical/social histories for residents, including healthy individuals and those with acute or chronic conditions.
- Make independent decisions regarding management and treatment of identified medical problems.
- Order, perform, and interpret laboratory tests for screening and diagnosis.
- Prescribe and order appropriate diet, medications, physical therapy, and other treatments.
- Perform therapeutic or corrective measures, including urgent care.
- Refer residents to specialists, physicians, or other healthcare providers as needed.
- Provide health maintenance, promotion, illness prevention, and medication counseling; evaluate effectiveness and adjust instruction as necessary.
- Initiate and maintain follow-up for noncompliant patients.
- Serve as a preceptor for medical, nursing, NP, or PA students.
- Consult with other healthcare providers about clients admitted to hospitals, home care, rehabilitation, or nursing homes.
- Correspond with insurers, employers, government agencies, and other providers regarding client care.
- Manage client care by developing treatment plans, monitoring progress, and determining when referrals or additional care (e.g., hospital admission) are necessary.
- Assess social/economic factors and tailor care accordingly; connect clients with community resources.
- Contract with clients regarding provider and client responsibilities.
- Maintain productivity levels to meet facility goals.
Qualifications
- Graduate of an accredited school of nursing with a Master’s Degree in Nursing.
- Nurse Practitioner certification in good standing.
- Minimum of two years as an RN in a healthcare setting.
- Current Ohio RN License and Nurse Practitioner license.
