Advanced Nurse Practitioner - Primary Care

About the Role

This position involves providing advanced clinical care as a Nurse Practitioner within a Whole Health System, focusing on personalized health plans for Veterans. The NP manages a patient panel with both acute and chronic conditions, emphasizing health promotion, disease prevention, and evidence-based practices.

Responsibilities

• Independently assess, diagnose, and manage patient care, including ordering and interpreting tests.
• Prescribe medications and non-pharmacologic interventions.
• Develop and implement individualized care plans to improve health outcomes.
• Collaborate with interdisciplinary teams and provide patient/family education.
• Refer to specialty services and adjust care plans as needed.
• Demonstrate leadership, professionalism, and adherence to core values of integrity, commitment, advocacy, respect, and excellence.

Requirements

• Master’s or doctoral degree as a Nurse Practitioner from an accredited program.
• Active, unrestricted NP license in the U.S.
• Board certification in Family, Adult, or Gerontology Nurse Practitioner from a recognized certifying body.
• Strong clinical, communication, and teamwork skills.
• Physical ability to perform assessments and related duties.

The role may include travel to clinics, providing gap coverage, and mandatory training, including women’s health.
