Nurse Practitioner-Primary Care

Join a trusted healthcare leader and make a lasting impact in the community. East Jefferson General Hospital, part of LCMC Health, is seeking a compassionate, patient-focused Primary Care Nurse Practitioner to join their growing outpatient team.
Responsibilities
- Deliver exceptional patient care
- Perform comprehensive health assessments, physical examinations, and preventive health screenings
- Diagnose and manage acute and chronic health conditions
- Develop individualized treatment plans and evaluate patient responses to care
- Educate patients and families on disease prevention, treatment options, and healthy lifestyle choices
- Respond promptly to patient calls, messages, and clinical concerns
- Maintain accurate and timely clinical documentation within the electronic medical record
- Navigate and respond appropriately to medical emergencies
- Identify and report signs of abuse, neglect, or other patient safety concerns in accordance with organizational policy
- Collaborate and lead with physicians, nurses, clinical staff, and leadership to ensure coordinated patient care
- Participate in daily huddles, provider meetings, and quality improvement initiatives
- Serve as a clinical resource and leader for the care team
- Contribute to practice operations and support a culture of continuous improvement
Qualifications
- Master's Degree from an accredited Nurse Practitioner Program required
- Current Registered Nurse (RN) license issued by the Louisiana State Board of Nursing
- Current Advanced Practice Registered Nurse (APRN) license issued by the Louisiana State Board of Nursing
- Basic Life Support (BLS) certification through the American Heart Association
