Duties
- The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers.
- This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest.
- As an employee operating in a Whole Health System of care - you will operate in a model with three core elements - seeking to create a personalized health plan for each Veteran.
- This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community.
- The Advanced Practice Nurse (APN) Ill Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission.
- The NP provides advanced clinical practice - consultation - and management.
- Duties include:
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
- Orders and interprets labs - tests - and diagnostic studies.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals and consults to specialty services.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
