Primary Care Advanced Practice Provider (NP OR PA)

Responsibilities

• Provide virtual primary care visits via telehealth, including assessments and chronic disease management
• Diagnose and manage chronic conditions and customize care plans
• Evaluate and treat acute concerns within the scope of virtual care
• Order and interpret lab tests and coordinate necessary follow-up care
• Manage medication prescriptions, ensuring adherence and safety
• Offer health coaching and education to empower patient self-management
• Facilitate referrals and coordination with community resources as needed

Qualifications

• Active Nurse Practitioner (NP) license in good standing in existing or future states or ability to obtain licensure in multiple states within 90 days of hire
• Active RN and APRN licenses, or Physician Assistant licenses, in good standing
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
• Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably in telehealth or virtual care settings
• Active BLS certification at time of hire
• Flexibility to work variable shift times aligning with patient time zones and potential weekend shifts based on needs

Pay

$120,000 – $155,000 annually
