Prelicensed Medical Social Worker - Dialysis

Role: Prelicensed Social Worker supporting patients with chronic kidney disease in a dialysis setting. Responsibilities: Conduct psychosocial assessments, assist in care planning, provide counseling and crisis intervention, and connect patients and families with community resources. Collaboration: Work closely with nurses, physicians, and interdisciplinary team members to address social and emotional needs, promote treatment adherence, and resolve barriers to care.
