Prelicensed Social Worker

Role Overview: Provides psychosocial services to patients undergoing dialysis, working within an interdisciplinary team to promote adjustment, rehabilitation, and improved quality of life. Supports staff in understanding the emotional and behavioral impact of Chronic Kidney Disease and participates in quality improvement initiatives to meet patient satisfaction and outcome goals. This is an entry-level MSW position. Key Responsibilities Patient Assessment & Care Planning: Evaluates psychosocial status, collaborates on care plans, offers counseling, and addresses barriers to treatment adherence and transplant referrals.Patient Education: Provides tailored education on kidney disease, treatment options, and psychosocial issues, ensuring patient understanding of rights and responsibilities.Support & Counseling: Offers supportive counseling, assists with community resource referrals, and supports advance directives discussions.Quality & Compliance: Participates in quality reviews, reports on adherence and quality indicators, and maintains documentation per policies.Patient Admission & Continuity of Care: Reviews patient needs, supports scheduling, and addresses discharge concerns.Insurance & Financial Assistance: Acts as liaison with insurance and billing departments, educating patients about coverage and payment options.Staff Collaboration & Training: Assists with staff hiring, training, and participates in team meetings to address urgent patient issues.
