Prelicensed Medical Social Worker – Dialysis

Role: Prelicensed Social Worker supporting patients with chronic kidney disease in a dialysis setting. Responsibilities Conduct psychosocial assessments and assist with care planning.Provide counseling and crisis intervention to patients and families.Connect patients with community resources to support their needs.Collaborate with nurses, physicians, and interdisciplinary team members to address social and emotional concerns.Support treatment adherence and help overcome barriers to care.
