Responsibilities

• Develop, implement, and document individualized care plans in collaboration with the healthcare team, patients, and families, ensuring effective communication across the continuum of care.
• Assess physical, psychosocial, emotional, cultural, and spiritual needs, considering patient preferences and barriers to involvement.
• Educate patients and families on managing their health, medications, and post-discharge care.
• Participate in safety initiatives and maintain compliance with regulatory standards.

Qualifications

• Required: RN license, Nursing Diploma or Associate's degree.
• Preferred: Bachelor's degree in Nursing, with at least 2 years of experience.
• Safety-sensitive position requiring constant alertness.
