Population Health PharmD Fellow/Instructor - School of Pharmacy

Position Summary

MCW School of Pharmacy is seeking applications for a Population Health Fellow. This individual will be appointed as a full-time, year-round non-tenure track Clinical Instructor. This position is a 1-year post-graduate opportunity, designed to prepare pharmacists for a career in population health management program design, implementation, and maintenance with an anticipated start date of 7/1/2027. This position reports to the Population Health Fellowship Director in the School of Pharmacy. Individuals that have completed PGY1 or PGY2 training program are encouraged to apply. The training year will consist of approximately 40% clinical practice in Population Health Management at Froedtert & 60% scholarship, teaching, and other academic-related duties. The fellow will have a longitudinal academia pharmacy experience, exposing them to a variety of teaching responsibilities, including lecturing, facilitating small group-based learning, course coordination and student assessment. Highlights of the training program include working with a grant implementation team on the design, implementation, clinical practice, and continuous quality improvement of a population health management program in hypertension. At the end of the training program, academic fellows will be well prepared for a career in population health management in an academic medical center.

Position Highlights

Clinical Practice

• Practice as a clinical pharmacist working at the top of license, operating under a collaborative practice agreement in an academic medical center
• Improve hypertension outcomes by practicing under a novel remote patient monitoring initiative in Southeastern Wisconsin utilizing self-measured blood pressure readings
• Participate in the continuous quality improvement of a population health management initiative by coordinating clinical care across different healthcare organizations, optimizing workflows, and collaborating with other healthcare personnel
• Collect and communicate operational metrics and quality measure performance with health system and grant team leadership

Scholarship

• Complete the MCW School of Pharmacy Research Certificate Program
• Complete a scholarly project related to population health, either through a quality improvement project, research project, or scholarship of teaching, learning and/or education
• Present research data at a professional meeting and the Annual MCW School of Pharmacy Research Symposium annually
• Submit paper for publication (novel clinical practice model, educational innovation, etc.)
• Represent the college at selected professional meetings (regional and national presence)

Teaching and College Service

• Design, implement, facilitate, and deliver innovative educational content within the Doctor of Pharmacy program
• Develop skills in educational training through a variety of classroom experiences (i.e. didactic lecturing, facilitating skills labs, and small group active-learning environments)
• Participate in a longitudinal specialty area of academia (i.e. Assessment, Curriculum, Admissions, Faculty Affairs, etc.)
• Develop skills necessary for successful course coordination, precept Introductory and/or Advanced Pharmacy Practice students on clinical rotations
• Engage in mentorship activities through student organizations and community service
• Exhibit professionalism through participation in pharmacy and interprofessional organizations and serving as student organization mentor
• Complete the MCW School of Pharmacy Teaching Certificate Program
• Precept pharmacy students on IPPE and APPE rotations during clinical practice days

Qualifications

• Applicants must hold a PharmD degree from an accredited U.S. program (or equivalent)
• Individuals who have completed a PGY1 pharmacy residency or a PGY-2 specialty residency or a fellowship and are interested in pursuing a career in population health management in an academic medical center are highly encouraged to apply
• Applicants without a PGY-1 residency will be interviewed based on their academic performance/experience
• All applications must be eligible for Wisconsin licensure and obtain Wisconsin licensure within 90 days of the offer
