About the role
Join Palo Alto Foundation Medical Group (PAFMG), one of the nation's premier physician-led, multi-specialty medical groups, as a Board Certified/Board Eligible Palliative Care Physician in Sunnyvale, California.
Responsibilities
- Work in a collaborative, multidisciplinary environment dedicated to improving the quality of life for patients and families facing serious illness.
- Deliver care wherever patients need it most—including the clinic, home, assisted living communities, skilled nursing facilities, and through telemedicine.
- Collaborate with experienced primary care physicians, specialists, nurses, social workers, chaplains, and other interdisciplinary team members.
- Develop comprehensive care plans, provide patient and family education, and deliver ongoing symptom management and supportive care.
Requirements
- Fellowship training in Hospice and Palliative Care Medicine.
- Board Certified or Board Eligible in Hospice and Palliative Care Medicine.
- Eligible for California medical licensure.

Schedule
