Physician Leader (MD /DO) Value-based Primary Care

About the Role

Value-based Physician Leader with a growing Troy/Detroit Medical Group team. As a Physician Leader, you will spend half the week managing chronic conditions of a small patient panel and half the week leading high-risk huddles and providing clinical oversight of Advanced Practice Providers (APPs).

Responsibilities

• Act as the clinical leader responsible for the performance of the clinical pod team, including accurate diagnosis and documentation, quality, patient experience, affordability, patient safety, clinical team dynamics, and provider productivity.
• Develop and maintain close working relationships with all clinical team members in the pod.
• Serve as the collaborating physician for associated pod Advanced Practice Providers (APPs).
• Partner with the Market Manager and market office staff in a dyad leadership model.
• Act as on-site supervising Physician for assigned APPs and Physicians (collaborating physician), ensuring compliance with state regulations.
• Participate in at least one ride-along with all collaborative APPs per quarter.
• Provide complex patient visits as needed for each APP under supervision.
• Collaborate with clinical team members (Community Health Workers, Medical Social Workers, Nurse Navigators, Care Managers, Patient Care Coordinators, etc.).
• Develop and participate in APP clinical development (evidence-based care, professionalism, etc.).
• Lead and participate in high-risk huddles, ensuring documented care plans for all patients discussed.
• Build a team-based culture and work collaboratively with clinical team members.
• Conduct 10 chart audits per month or per state regulations, whichever is higher.
• Act as a resource for clinical team members regarding patient care questions/concerns and participate in difficult conversations with patients/family members.
• Routinely conduct panel reviews with APPs.
• Sign DME, death certificates, and other orders as needed on behalf of APPs (as regulated by state).
• Frequently review Quality metrics and pod-level performance to identify areas for improvement.
• Review clinician capacity and productivity performance; partner with Site Medical Directors to develop countermeasures when providers are off track.
• Work collaboratively with the Regional Medical Director (RMD) to identify opportunities for care improvement.
• Assist with service inquiries, resolution of patient complaints, and requests for the pod.
• Help pod providers achieve higher EMR efficiency.
• Participate in patient care for assigned panel and other patients as needed (high-risk or complex patients, cognitive assessments, etc.).
• Lead clinical education on accurate diagnosis, documentation, and management of chronic conditions.
• Assist with compliance and HR resolutions.
• Responsible for discipline actions and performance reviews for direct reports.
• Perform additional duties as assigned by the Regional Medical Director related to clinical pod performance and outcomes.

Requirements

• Bachelor’s Degree
• Doctor of Medicine or Osteopathy
• Valid driver’s license and good driving record
• Active CPR Certification
• Unrestricted DEA
• Completion of an accredited Residency Program
• Unrestricted Medical License
• Board certification required for all newly hired or promoted physicians on or after January 1, 2026; must obtain and maintain certification in accordance with organizational and applicable regulatory standards.

Preferred Qualifications

• 3 years of experience as a Physician collaborating with Advanced Practice Providers
• Experience in Geriatrics or with complex patients
