Key Responsibilities
- Provide physical therapy treatments as directed by the supervising Physical Therapist
- Follow physician orders and individualized treatment plans
- Document daily treatments, weekly progress notes, and resident outcomes accurately and timely
- Participate in patient care conferences and rehabilitation meetings as needed
- Monitor resident progress and communicate updates to the Physical Therapist
- Assist with cleaning and maintenance of therapy equipment and treatment areas
- Train and assist Restorative Aides in carrying out therapy-related programs
- Communicate effectively with residents, families, nursing staff, and interdisciplinary team members
- Participate in staff education and in-service training programs
- Educate residents, families, and caregivers on maintenance programs and discharge recommendations
- Report equipment concerns and assist in maintaining a safe therapy environment
- Ensure compliance with all facility policies, state regulations, and payer requirements
Qualifications
- Graduate of an accredited Physical Therapist Assistant program
- Experience in skilled nursing, rehabilitation, or long-term care preferred
- Experience with PointClickCare (PCC) and Casamba preferred
- Strong communication, documentation, and organizational skills
- Licenses and certifications:
- Current South Carolina Physical Therapist Assistant (PTA) license in good standing
- Maintain all required continuing education requirements
- Remain in good standing with the applicable State Board
