About the role

Transform lives as a licensed Physical Therapist providing in-home therapy services in the beautiful Grand Junction area.

Responsibilities

• Deliver skilled physical therapy services tailored to individual patient needs.
• Perform evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
• Develop and implement personalized physical therapy plans based on physician orders.
• Provide skilled interventions:
  • Gait training
  • Balance training
  • Therapeutic exercises
  • Mobility training
  • Fall prevention strategies
• Empower patients and caregivers by:
  • Educating them on treatment plans and home exercise programs.
  • Sharing safety strategies to promote independence.
• Collaborate with the interdisciplinary care team:
  • Communicate patient progress and any changes in condition.
  • Support clinical oversight of Physical Therapist Assistants (PTAs) as permitted by Colorado law.
• Ensure top-notch documentation:
  • Complete accurate, timely records in Homecare Homebase (HCHB).
  • Comply with Medicare Conditions of Participation and agency standards.
  • Maintain HIPAA compliance and all privacy regulations.
• Participate in care coordination efforts:
  • Work closely with agency clinical staff, including RNs and other therapists.
  • Ensure efficiency in patient care and regulatory adherence.

Requirements

• Current, unrestricted Colorado Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• Valid driver’s license and reliable transportation.
• Ability to practice independently in a home-based setting.
• Proficiency or willingness to learn Homecare Homebase (HCHB).

Qualifications

Preferred:

• Experience in home health.
• Familiarity with Medicare documentation and home health regulations.
• Experience working with geriatric and medically complex populations.
