About the Role
We are seeking a Physical Therapist (PT) to provide high-quality home health services to patients across the New Haven to Greenwich coverage area. Visits are scheduled in close geographic proximity to support efficient patient care and travel.
Responsibilities
- Complete PT admissions and initial evaluations
- Perform resumption of care (ROC) visits and recertifications
- Provide routine physical therapy treatment visits
- Develop and implement individualized treatment plans
- Complete all clinical documentation within 24 hours of each visit
- Collaborate with physicians, caregivers, and interdisciplinary teams to ensure optimal patient outcomes
Requirements
- Doctorate Degree in Physical Therapy (DPT)
- Active Physical Therapist license
- Strong clinical assessment and patient care skills
- Home health experience preferred
Preferred Qualifications
- Homecare Homebase (HCHB) experience
- OASIS documentation experience
