About the Role
Licensed Physical Therapist delivering personalized geriatric care to older adults in private residences and senior living communities.
Responsibilities
- Conduct evaluations and develop individualized treatment plans
- Provide evidence-based therapy sessions to improve mobility and independence
- Document care accurately and collaborate with interdisciplinary teams
- Educate patients and caregivers
- Incorporate telehealth when appropriate
- Offer one-on-one therapy and support patient wellness
- Work within diverse care settings and promote safety and aging in place
Requirements
- Valid Physical Therapist license
- Degree from an accredited Physical Therapy program
- Proficiency in Electronic Medical Records (EMR) and basic computer skills
- Current CPR certification
- Ability to meet physical demands such as lifting, repositioning, and driving
