About the role

We are seeking a full-time Physical Therapist to provide direct rehabilitation care to patients in a skilled nursing facility setting.

Responsibilities

• Implementing and adjusting individualized treatment plans based on physician referrals
• Overseeing and evaluating care provided by PTAs, rehab aides, and students
• Charting accurately and utilizing therapy software effectively
• Establishing, assessing, and modifying realistic patient goals
• Collaborating with an interdisciplinary team to deliver comprehensive care
• Providing excellent patient service and maintaining professionalism and confidentiality
• Participating in community health projects and adhering to safety and compliance standards

Qualifications and skills

• Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT)
• Current licensure in applicable state, maintaining active licensure
• CPR certification required; new grads are welcome to apply
• Proficiency in Microsoft Word, Excel, and email communication
• Strong communication, decision-making, and body mechanics skills
• Knowledge of post-acute care regulations and safety procedures
