Physical Therapist (PT)

About the role

Fair Oaks Lodge, a skilled nursing facility in the Eden Senior Care Network, is now hiring a Physical Therapist to provide rehabilitation services in one or more of our service locations. Eden Senior Care, founded in 2016, is a growing Healthcare Management company focused on managing and operating Skilled Nursing, Rehabilitation and Assisted Living communities in Minnesota, Wisconsin, Illinois, Ohio, Pennsylvania, Indiana and Alabama.

Responsibilities

• Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
• Identify need for and make referral to other disciplines to address the comprehensive needs of the patient
• Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathology
• Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards

Qualifications

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy (MPT) from an accredited program
• Active Minnesota Physical Therapy (PT) license in good standing
• Experience in skilled nursing, home health, or rehabilitation settings preferred
