Pay

Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist (PT) in Home Health

• You’ll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections
• Play a key role in helping clients achieve their highest level of independence and well-being

Provide Direct Client Care

• Evaluate clients by obtaining and interpreting subjective and objective data
• Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders
• Deliver high-quality care that empowers clients to achieve their functional goals
• Perform re-evaluations as required to assess progress and adjust care plans accordingly
• Maintain Accurate Client Records and Billing - Document all services accurately and promptly using Vivie’s EMR system
• Demonstrate a thorough understanding of all payer sources and authorization requirements
• Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines
• Recommend and oversee continuation programs to support long-term client’s success and optimize discharge outcomes
• Make referrals to continuation programs as necessary to promote maximum independence and functionality

Communicate Effectively and Collaboratively

• Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team
• Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery

• A current Physical Therapist license in the state of Minnesota

Qualifications

• Strong collaboration and teamwork skills
