Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist (PT) in Home Health
- You’ll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections
- Play a key role in helping clients achieve their highest level of independence and well-being
Provide Direct Client Care
- Evaluate clients by obtaining and interpreting subjective and objective data
- Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders
- Deliver high-quality care that empowers clients to achieve their functional goals
- Perform re-evaluations as required to assess progress and adjust care plans accordingly
- Maintain Accurate Client Records and Billing - Document all services accurately and promptly using Vivie’s EMR system
- Demonstrate a thorough understanding of all payer sources and authorization requirements
- Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines
- Recommend and oversee continuation programs to support long-term client’s success and optimize discharge outcomes
- Make referrals to continuation programs as necessary to promote maximum independence and functionality
Communicate Effectively and Collaboratively
- Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team
- Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery

- A current Physical Therapist license in the state of Minnesota
Qualifications
- Strong collaboration and teamwork skills
