Pay

Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

About the role

As a Physical Therapist (PT) in Home Health, you’ll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections. Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you’ll play a key role in helping clients achieve their highest level of independence and well-being.

Responsibilities

  • Evaluate clients by obtaining and interpreting subjective and objective data.
  • Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders.
  • Deliver high-quality care that empowers clients to achieve their functional goals.
  • Perform re-evaluations as required to assess progress and adjust care plans accordingly.
  • Maintain accurate client records and billing.
  • Demonstrate a thorough understanding of all payer sources and authorization requirements.
  • Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.
  • Recommend and oversee continuation programs to support long-term client’s success and optimize discharge outcomes.
  • Make referrals to continuation programs as necessary to promote maximum independence and functionality.
  • Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team.
  • Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.

Requirements

  • Current Physical Therapist license in the state of Minnesota.

Qualifications

  • Strong collaboration and teamwork skills.
  • Clear and compassionate communication abilities.
  • Proficient in English for effective communication with staff, clients, and visitors.

Skills

  • Successful completion of a state-mandated background check.
  • Able to physically perform the duties required by the position.
  • Must obtain CPR certification within 6 months of hire.
