Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

Deliver Direct Client Care

• Conduct comprehensive evaluations and develop personalized treatment plans with short-term and long-term goals
• Maintain accurate client records
• Document services accurately and timely using Vivie’s EMR system, ensuring compliance with all payer sources and authorization requirements
• Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes
• Collaborate and communicate effectively
• Work closely with clients, families, physicians, and the interdisciplinary team to provide care
• Ensure regulatory compliance

Job Requirements

• Current Physical Therapist license in the state of Minnesota
• Strong collaboration and teamwork skills
• Clear and compassionate communication abilities
• Proficient in English for effective communication with staff, clients, and visitors
• Successful completion of a state-mandated background check
• Able to physically perform the duties required by the position
• Must obtain CPR certification within 6 months of hire
