Physical Therapist (PT) - Outpatient Rehab

About the role

The Physical Therapist (PT) at Hartford HealthCare Rehabilitation Network (HHCRN) plays a crucial role in evaluating, determining treatment strategies, delivering, and delegating services to maximize patient's functional independence. This position focuses on physical dysfunction, with emphasis on strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait, and functional abilities in activities of daily living.

Responsibilities

• Evaluates and examines patients to assess their level of function, including strength, movement, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, and vital signs.
• Develops an individualized patient-centered treatment plan of care to promote optimal movement function and safety.
• Implements, delivers, and delegates patient care by utilizing appropriate interventions and clinical decision-making, including modalities such as heat, cold, light, water, sound, electricity, mobilization, and therapeutic exercise.
• Coordinates patient care with other care providers such as Rehab Therapists, Nurses, Social Workers, and participates in patient transition planning and/or discharge planning, recommending community resources to meet patient needs.

Qualifications

• Graduate of a Physical Therapy Program from an accredited American Physical Therapy Association school, with a BS, master's, or doctorate degree.
• CT active or temporary license.
• CPR certification.
• Will consider new graduates.
