About the Role

Provides comprehensive physical therapy evaluation and treatment for patients of all ages, adapting care plans based on patient response and current evidence. Acts as a clinical resource and role model for staff and students, ensuring safe patient transfers and promoting proper body mechanics. Designs personalized care plans with clear goals, outcomes, and discharge strategies. Educates patients, families, caregivers, and healthcare team members effectively.

Responsibilities

• Performs examinations, assesses clinical findings, and documents according to policies.
• Uses support personnel ethically, monitoring and providing feedback on care delivery.
• Completes objective testing, applies therapeutic modalities, and communicates progress to providers.
• Evaluates orthopedic and neurologic impairments, performs daily documentation, and applies stabilization techniques.
• Emphasizes dignity, respect, safety, and evidence-based practice in all duties.

Requirements

• Bachelor’s degree in Physical Therapy.
• Licensure as a Physical Therapist (PT) in Illinois.
• Basic Life Support (BLS) certification.
• Must complete ongoing education related to acute stroke care and adhere to clinical standards.
