H2 Health is seeking a Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our outpatient team in Dublin, Virginia. At H2 Health, you'll work with a diverse outpatient population while providing individualized, evidence-based care. Our clinicians treat orthopedic injuries, post-surgical conditions, musculoskeletal disorders, balance and vestibular impairments, and mobility-related challenges. You'll have the opportunity to build meaningful relationships with patients while collaborating with a supportive clinical team dedicated to helping individuals achieve their rehabilitation goals.

Responsibilities

Physical Therapist (PT)

  • Perform comprehensive patient evaluations and develop individualized plans of care
  • Deliver evidence-based treatment interventions to improve mobility, strength, balance, function, and overall quality of life
  • Treat a variety of orthopedic, post-operative, musculoskeletal, balance, vestibular, and mobility-related conditions
  • Collaborate with patients, families, physicians, and interdisciplinary team members to achieve optimal outcomes
  • Educate patients on injury prevention, wellness, and home exercise programs
  • Maintain accurate and timely documentation of patient progress and treatment outcomes
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management
  • Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians
  • Contribute to a positive clinic culture focused on patient-centered care and clinical excellence

Physical Therapist Assistant (PTA)

  • Provide skilled physical therapy treatment under the direction of a licensed Physical Therapist
  • Implement individualized treatment plans to improve mobility, strength, balance, and functional independence
  • Treat orthopedic, post-operative, musculoskeletal, balance, vestibular, and mobility-related conditions
  • Educate patients and caregivers on home exercise programs and injury prevention strategies
  • Accurately document patient progress and communicate treatment updates with the clinical team
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management
  • Collaborate with Physical Therapists and support staff to support strong patient outcomes
  • Work within a manageable caseload environment, typically targeting approximately 10-12 patient visits per day for full-time outpatient clinicians
  • Contribute to a positive clinic culture focused on patient-centered care and clinical excellence

Requirements

Physical Therapist (PT)

  • Doctor of Physical Therapy (DPT) degree or equivalent legacy degree (MSPT/MPT/BPT) from a CAPTE-accredited Physical Therapy program
  • Current Virginia Physical Therapist license or eligibility for licensure
  • CPR certification or willingness to obtain as required
  • New graduates are encouraged to apply

  • Associate degree from a CAPTE-accredited Physical Therapist Assistant program
  • Current Virginia Physical Therapist Assistant license or eligibility for licensure
  • CPR certification or willingness to obtain as required
  • New graduates are encouraged to apply
