Physical Therapist (PT) - Mobile Outpatient

The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in an independent living facility. Powerback Rehabilitation To You allows the Physical Therapist to evaluate and treat in the most functional environment, which is the patient's home.

Responsibilities

• Supervision of Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Adherence to department and patient treatment schedule, timely clinical and billing documentation requirements, as well as work-related meetings.
• Attendance and contribution to patient care, staffing conferences, and other related meetings.
• Referrals to other disciplines and agencies to address the comprehensive needs of the patient.
• Consultation with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
• Adaptation of program and/or plan of care according to the needs of the individual patient.
• Promotion of a safe environment and enforcement of the elimination of fire and safety hazards.
• Ordering of supplies and equipment as necessary.
• Maintenance, identification, and reporting of the need for repair of equipment.
• Prioritization of patient service to ensure patients and families receive the highest quality of service in a caring and compassionate atmosphere recognizing individual needs and rights.
• Screening, examining, and evaluating patients, including history, systems review, and application of appropriate tests and measures. Synthesis of information and interpretation of examination findings to establish a diagnosis, identify impairments, determine predicted level of improvement, and design a plan of care.
• Development of appropriate treatment goals and methods in collaboration with the patient and caregivers, implementation of the physical therapy treatment plan, and completion of all related documentation and record keeping. Services include but are not limited to:
  • Therapeutic exercise (aerobic capacity conditioning, balance coordination, agility training, body mechanics, postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training)
  • Functional training
  • Manual therapy
  • Airway clearance techniques
  • Integumentary repair and protection
  • Use of electrotherapeutic, physical agent, and mechanical modalities
  • Health and Wellness
  • Education on Chronic Disease Management
  • Falls Assessment and Interventions to reduce fall risk
  • Case Management in the home and community
• Design, fabrication, fitting, and/or application of adaptive devices, and training patients in their use, including orthotics and prosthetics.
• Selection of appropriate activities suited to the individual's physical and mental capacity, intelligence level, and interest.
• Instruction of patients in the skills and techniques required for participation in mobility activities and evaluation of patient progress, attitudes, and behavior as related to rehabilitative potential.
• Performance of routine reexamination as needed/required to modify/progress the plan of treatment.
• Incorporation of health and wellness programming into treatment plans for patients.
• Performance of home and community mobility assessments and provision of recommendations for safe mobility in the home and community space.
• Performance of other related duties as required.

Requirements

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy; or
• Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or
• Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
• Valid driver's license or reliable, independent transportation to and from patients' residences.

Pay

Posted Salary Range: USD $79,000.00 - USD $95,000.00 per year.

Bonus: USD $5,000.00
