About the role
Powerback Rehabilitation To You allows the Physical Therapist to evaluate and treat older adults in the most functional environment—their own home (private residence or independent-living facility). You will develop and deliver individualized therapy programs, collaborate with other disciplines, and educate patients and caregivers to promote independence, health, and safety.
Responsibilities
- Supervise Physical Therapy Assistants, students, and support personnel in accordance with state licensure and professional standards.
- Adhere to department and patient treatment schedules, complete timely clinical and billing documentation, and attend work-related meetings.
- Participate in patient-care and staffing conferences; make referrals to other disciplines and agencies to address comprehensive patient needs.
- Consult with the rehabilitation team to select and adapt therapeutic regimens based on each patient’s needs and capabilities.
- Screen, examine, and evaluate patients; establish diagnoses, impairments, and functional goals; design and implement treatment plans.
- Provide therapeutic techniques including:
- Therapeutic exercise (aerobic conditioning, balance, coordination, agility, body mechanics, flexibility, gait, neuromotor development, relaxation, strength, power, endurance)
- Functional training
- Manual therapy
- Airway clearance techniques
- Integumentary repair and protection
- Electrotherapeutic, physical-agent, and mechanical modalities
- Educate patients on chronic-disease management, falls assessment, and interventions to reduce fall risk.
- Perform home and community mobility assessments; recommend modifications for safe mobility.
- Design, fabricate, fit, and train patients in adaptive devices, orthotics, and prosthetics.
- Instruct patients in mobility skills and evaluate progress, attitudes, and behavior.
- Perform routine re-examinations to modify or progress the treatment plan.
- Incorporate health and wellness programming into treatment plans.
- Maintain and order supplies and equipment; ensure a safe environment by eliminating fire and safety hazards.
- Provide compassionate, patient-centered care that respects individual needs and rights.
Qualifications
- Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
- One of the following:
- Master’s degree in Physical Therapy; or
- Master’s degree in a Physical Therapy–related field plus a Bachelor’s degree in Physical Therapy; or
- Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
- Foreign Physical Therapy degree must be evaluated as equivalent to a U.S. degree in Physical Therapy.
- Valid driver’s license or reliable, independent transportation to and from patients’ residences.

Pay
USD $79,000.00 – USD $110,000.00 per year.
