The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in an independent living facility. Powerback Rehabilitation To You allows the Physical Therapist to evaluate and treat in the most functional environment, which is the patient's home.

• Supervision of Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Adherence to department and patient treatment schedule, timely clinical and billing documentation requirements, as well as work-related meetings.
• Attendance and contribution to patient care, staffing conferences, and other related meetings.
• Referrals to other disciplines and agencies to address the comprehensive needs of the patient.
• Consultation with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
• Adaptation of program and/or plan of care according to the needs of the individual patient.
• Promotion of a safe environment and enforcement of the elimination of fire and safety hazards.
• Ordering of supplies and equipment as necessary.
• Maintenance, identification, and reporting of the need for repair of equipment.
• Ensuring patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes individuals' needs and rights.
• Screening, examining, and evaluating patients, including history, systems review, and application of appropriate tests and measures.
• Synthesis of information and interpretation of examination findings to establish a diagnosis, identify impairments, determine the predicted level of improvement, and design a plan of care.
• Development of appropriate treatment goals and methods in collaboration with the patient and caregivers, implementation of the physical therapy treatment plan, and completion of all related documentation and record keeping.
• Application of therapeutic techniques in areas such as:
  • Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training.
  • Functional training.
  • Manual therapy.
  • Airway clearance techniques.
  • Integumentary repair and protection.
  • Use of electrotherapeutic, physical agent, and mechanical modalities.
  • Health and Wellness.
  • Education on Chronic Disease Management.
  • Falls Assessment and Interventions to reduce fall risk.
  • Case Management in the home and community.
• Design, fabrication, fitting, and/or application of adaptive devices, and training patients in the use of adaptive devices including orthotics and prosthetics.
• Selection of appropriate activities suited to the individual's physical and mental capacity, intelligence level, and interest.
• Instruction of patients in the skills and techniques required for participation in mobility activities and evaluation of patient progress, attitudes, and behavior as related to rehabilitative potential.
• Performance of routine reexamination as needed/required to modify/progress the plan of treatment.
• Incorporation of health and wellness programming into treatment plans for patients.
• Performance of home and community mobility assessments and provision of recommendations for safe mobility in the home and community space.
• Performance of other related duties as required.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• Master's degree in Physical Therapy; or
• Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or
• Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
• Valid driver's license or reliable, independent transportation to and from the patients' residence.

Pay

Posted Salary Range: USD $47.00 - USD $63.00 /Hr. Bonus: USD $5,000.00
