Physical Therapist Mobile Outpatient

About the Role

Powerback Rehabilitation is seeking a full-time Physical Therapist to provide outpatient rehabilitation services in an in-home setting for the older adult population. This role allows you to evaluate and treat patients in their most functional environment—their home—whether that’s a private residence or an independent living facility. You’ll develop and deliver individualized therapy programs to restore, reinforce, and enhance patient performance while promoting independence and productivity.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Adhere to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings.
• Attend and contribute to patient care, staffing conferences, and other related meetings.
• Make referrals to other disciplines and agencies to address the comprehensive needs of the patient.
• Consult with members of the rehabilitation team to select appropriate therapeutic regimens based on patient needs and capabilities.
• Adapt programs and plans of care to meet the individual needs of each patient.
• Promote a safe environment and enforce the elimination of fire and safety hazards.
• Order and maintain supplies and equipment, and report repair needs as necessary.
• Prioritize patient service, ensuring a caring and compassionate atmosphere that recognizes individual needs and rights.
• Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
• Develop and implement individualized treatment plans, including goal-setting in collaboration with patients and caregivers.
• Provide therapeutic techniques such as:
  • Therapeutic exercise (aerobic capacity conditioning, balance coordination, agility training, body mechanics, flexibility, gait training, neuromotor development, relaxation, strength, power, and endurance training).
  • Functional training.
  • Manual therapy.
  • Airway clearance techniques.
  • Integumentary repair and protection.
  • Use of electrotherapeutic, physical agent, and mechanical modalities.
• Provide health and wellness education, including chronic disease management and falls assessment/interventions.
• Design, fabricate, fit, and train patients in the use of adaptive devices, including orthotics and prosthetics.
• Perform home and community mobility assessments and provide recommendations for safe mobility.
• Perform routine reexaminations to modify or progress the treatment plan as needed.
• Perform other related duties as required.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
• One of the following educational requirements:
  • Master’s degree in Physical Therapy; or
  • Master’s degree in a Physical Therapy-related field plus a Bachelor’s degree in Physical Therapy; or
  • Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• Degrees earned outside the U.S. must be evaluated as equivalent to a U.S. Physical Therapy degree.
• Valid driver’s license or reliable, independent transportation to and from patients’ residences.

Pay

Posted salary range: USD $79,000.00 – USD $110,000.00 per year, plus a USD $5,000.00 bonus.
