Job Description
What You’ll Do:
- Cook up care for your assigned patient caseload, including delegating to and supervising PTA staff (if applicable), processing patient condition change visits, and completing OASIS time points.
- Build a solid foundation for your patient and family, conducting a thorough initial assessment to identify home care needs. Perform a comprehensive physical assessment and gather a detailed history of their medical background to craft a personalized plan of care that includes therapeutic, preventive, and rehabilitative actions, involving the patient and family in the planning process.
- Construct and refine your patient's care plan as needed, implementing appropriate preventive and rehabilitative therapy procedures. Administer medications and treatments as directed by the physician.
- Stay in touch with the physician about your patient's needs and promptly relay any changes in their condition. Fetch/receive physician's orders as necessary.
- Keep all patient information confidential and adhere to agency and federal HIPAA guidelines.
- Master OASIS completion and document your work in line with agency timeliness policies.
What You’ll Need
- A degree from an accredited physical therapy program, with a DPT being preferred.
- A current and active New Hampshire Physical Therapist license.
- An American Health Association BLS certification.
- At least two years of physical therapy experience, with at least one year in public health or home care.
- Experience with OASIS is a plus.

Pay Range
$80,080.00 USD – $107,777.00 USD
