Case Manager
Responsibilities
- Assesses immediate needs of crisis shelter residents and making appropriate referrals, including referring all clients to health clinic for updated physical, coordinating with the hospital to schedule and reschedule appointments as necessary, and following up with the clients in regards to these appointments.
- Affords assistance in obtaining public benefits for which they are eligible (e.g. Medicaid, food stamps, housing, storage, SSI, and childcare subsidies).
- Completes shelter assessments as needed.
- Travel to the (3) dwellings to complete intakes and discharges as needed.
- Conduct apartment inspections at the three dwellings at least twice a month as needed.
- Conduct apartment inspections at Rosa Parks at least twice a month.
- Complete and submit packets NOVA packets for resident.
- Complete and submit ADVENT transfers.
- Affords assistance in obtaining self-sufficiency through appropriate referrals for counseling, financial counseling, ESOL classes, job readiness programs and higher education.
- Completes economic assessment with all clients.
- Affords accompanying clients to appointments as needed.
- Conducts preliminary inspection on resident’s prospective apartments.
- Completes, submits and follows up on housing packets with HRA and HPD.
- Refers clients to other appropriate legal and social services both within and outside of Sanctuary.
- Prepares requests for educational and financial assistance grants on behalf of clients.
- Advocates on behalf of clients for needed services.
- Maintains accurate case records, documents all services provided within Sanctuary’s Service Tracking System and prepares statistics for monthly reports.
- Affords assistance in issues related to children’s schooling including but not limited to enrolling children in school, requesting bussing from the DOE, requesting metro cards for transportation, communicating with the school for educational needs, or finding appropriate school placements.
- Filing documents and ensuring all case files are up to date.
- Coordinates special events and celebrations for clients.
- Provides backup coverage for Sanctuary’s hotline.
- Provides on-site coverage at Rosa Parks at least three days a week, more if needed.
- Provides on-site coverage in the absence of the Program Manager.
- Performs any other department or agency related duties or special projects as directed.
Requirements
- Associates Degree required, Bachelor’s Degree preferred.
- At least one (1) year of experience in the social services field, preferably in domestic violence.
- Fluency in both English and Spanish or an additional community language is strongly preferred.
- Strong verbal/written communication and interpersonal skills.
- Ability to work independently, multitask, and prioritize.
- Familiarity with Microsoft Office, including Outlook, Word, and Excel.
Budgeted Salary
Budgeted Salary: $29.73 - $32.84 per hour; (annualized based of 54,108.00-$ 59,769.00/yr); based on experience pursuant to wage scale in accordance with a collective bargaining agreement
Work position
Full-time (standard 35 hours/week)

Currently in-person/onsite full time - must be able to meet job location schedule obligations
