About the Role
The Physical Therapist (PT) provides direct rehabilitation services to patients in skilled nursing facilities, supporting long-term care and short-term recovery. The role involves developing and implementing treatment plans, assessing patient progress, and collaborating with a multidisciplinary team to enhance patient outcomes.
Responsibilities
- Follow established physical therapy treatment plans and modify goals as needed
- Assess and document patient progress accurately and timely
- Supervise and evaluate care provided by PT assistants, aides, and students
- Maintain patient safety, rights, and confidentiality in compliance with regulations
- Utilize therapy software and chart appropriately
- Communicate effectively with patients, families, and team members
- Participate in community health initiatives as appropriate
Qualifications & Skills
- Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT)
- Current state license in good standing; CPR certification required
- Experience preferred but new graduates are welcome
- Proficiency in Microsoft Office and good communication skills
- Ability to make independent decisions and demonstrate good body mechanics
- Knowledge of laws, regulations, and best practices in post-acute care
