About the Role

The Physical Therapist (PT) provides direct rehabilitation services to patients in skilled nursing facilities, supporting long-term care and short-term recovery. The role involves developing and implementing treatment plans, assessing patient progress, and collaborating with a multidisciplinary team to enhance patient outcomes.

Responsibilities

  • Follow established physical therapy treatment plans and modify goals as needed
  • Assess and document patient progress accurately and timely
  • Supervise and evaluate care provided by PT assistants, aides, and students
  • Maintain patient safety, rights, and confidentiality in compliance with regulations
  • Utilize therapy software and chart appropriately
  • Communicate effectively with patients, families, and team members
  • Participate in community health initiatives as appropriate

Qualifications & Skills

  • Graduate from an accredited physical therapy program (BSPT, MSPT, or DPT)
  • Current state license in good standing; CPR certification required
  • Experience preferred but new graduates are welcome
  • Proficiency in Microsoft Office and good communication skills
  • Ability to make independent decisions and demonstrate good body mechanics
  • Knowledge of laws, regulations, and best practices in post-acute care
