About the Role

Watertown Health Care Center is a skilled nursing home facility dedicated to providing residents with an exceptional experience through compassionate, respectful, and professional care. We are seeking a Physical Therapist to evaluate, treat, and document care for residents with impaired ability to perform activities of daily living. This role involves participating in interdisciplinary team meetings, developing restorative nursing programs, and helping residents maintain or attain the highest level of independence possible.

Responsibilities

  • Evaluate and document evaluations of referred residents in accordance with facility policies and procedures.
  • Provide appropriate treatment to help residents maintain or attain the highest level of function possible within their physical abilities.
  • Recommend and provide preventative measures and adaptive equipment to prevent decline in residents’ function.
  • Educate residents or caregivers to support functional independence.
  • Maintain timely and accurate resident reports as required by regulatory agencies and third-party payers.
  • Make recommendations for adaptive equipment and preventative measures to administration.
  • Observe infection control procedures and ensure resident safety under your supervision.
  • Complete documentation related to evaluations, treatment plans, MDS assessments, care plans, and progress notes.
  • Participate in resident care conferences, interdisciplinary care conferences, and rehabilitation conferences.
  • Contribute to programs improving resident care related to Quality Indicators and assume Quality Assurance duties as assigned.
  • Collaborate with residents, their representatives, facility staff, physicians, consultants, and ancillary service providers.
  • Adhere to all facility policies, safety guidelines, and the Corporate Compliance Program Code of Conduct.
  • Protect the privacy of resident Protected Health Information.
  • Perform all other duties as assigned.

Qualifications

  • Master’s degree in Occupational Therapy.
  • NBCOT Certified.
  • Excellent verbal and written communication and organizational skills.
  • Ability to read, analyze, and interpret medical information.
  • Effective communication with residents, families, and healthcare professionals.
  • Strong reasoning ability to define problems, collect data, establish facts, and draw conclusions.
  • Proficiency in software such as Microsoft Word and Excel.
