Physical Therapist (PT) - Assisted Living Facility Based Home Health

Responsibilities

• Supervise, evaluate, and provide therapy services to clients in various senior living communities.
• Work closely with the assisted living facility's wellness team to maintain strong lines of communication and continuity of care.
• Examine and treat patients with physical impairments using physical modalities.
• Aid in the improvement of mobility, function, independence in self-care, and daily living skills for patients.
• Teach family members and other members of the health care team about the care provided to clients.
• Administer skilled care to clients requiring intermittent professional services as per physician’s orders and the established plan of care.

Qualifications

• Currently licensed as a Physical Therapist (PT) in the state of employment.
• A minimum of one year of clinical experience is preferred.
• Demonstrate knowledge and skill in current therapy practice.
• Possess a valid state driver’s license and automobile liability insurance.
