How You'll Make a Difference
Care and compassion are at the heart of what you will do as a Physical Therapist (PT). You will be part of an interdisciplinary team focused on delivering high-quality, patient-centered care and achieving positive outcomes. In this role, you will provide home-based therapy services as directed by the attending physician, working closely with patients and their families throughout the recovery process. Home care offers valuable context and real-world insight into what will best support patients in restoring their health and independence.

How You'll Work
- Deliver skilled physical therapy services in accordance with physician orders, ensuring patient safety while providing education to both patients and their family members.
- Maintain accurate and compliant clinical documentation in alignment with all applicable regulations and standards.
- Plan of Care: Provide skilled physical therapy as ordered by the attending physician. Observe, document, and report patient responses, progress, and any changes in condition. Conduct OASIS assessments to establish and adjust individualized plans of care as needed.
- Minimize Patient Risk: Reduce re-hospitalizations by prioritizing visits for high-risk patients, providing education on preventive measures, and promoting informed decision-making.
- Family Educator/Advocate: Educate patients and families on disease or injury management throughout all phases of therapy. Provide instruction on the use of mobility aids and adaptive equipment, including wheelchairs, braces, walkers, crutches, canes, and prosthetic or orthotic devices.
- Supervision: Supervise and oversee therapy services provided by Physical Therapist Assistants (PTAs) and, when appropriate, nurse aides.
- Communication/Collaboration: Maintain consistent communication with PTAs and nurse aides regarding supervisory visit findings and care coordination.
- Policies: Complete all clinical documentation in accordance with agency protocols and Medicare/Federal guidelines.
- Rules and Regulations: Adhere to all agency policies, procedures, and regulatory requirements. Communicate any changes in schedule or availability to appropriate team members.
- Operations: Participate in required in-service training and attend mandatory organizational meetings.

Requirements
- Current Physical Therapist license in the state of employment.
- Valid driver’s license and active auto insurance in your name.
