About the Role
At Pivot Home Health, you'll have the freedom to choose your caseload with no minimum visit requirement, set your own schedule, and provide therapy in homes for natural, meaningful progress. We also provide an iPad for seamless electronic documentation.
Responsibilities
- Provide physical therapy services in home and community settings
- Document patient progress electronically using an iPad
- Collaborate with interdisciplinary teams to develop and implement treatment plans
- Adapt therapy sessions to meet individual patient needs
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Active Colorado Physical Therapist (PT) license
- Current CPR certification
- Strong communication, organization, and collaboration skills
- Passion for helping patients across the lifespan to reach their full potential through home-based care
Qualifications
- New graduates and relocating clinicians encouraged to apply

Pay
$80 - $90 per regular visit
