Physical Therapist - Per Visit

Role Overview

The Physical Therapist in Home Health evaluates patients' functional status, mobility, and medical conditions to develop and implement personalized treatment plans.

Responsibilities

• Assess patient needs and assist in creating therapy plans in collaboration with physicians.
• Provide treatment to improve function, relieve pain, and maintain maximum mobility within legal practice scope.
• Monitor and report patient responses and condition changes to healthcare team members.
• Educate patients, families, and caregivers about care plans, limitations, and progress.
• Complete evaluations, develop plans of care, and document progress during visits.
• Operate effectively in diverse home environments, ensuring equitable care regardless of patient background.

Qualifications & Skills

• Current state licensure in Physical Therapy.
• Valid driver's license, vehicle insurance, or access to transportation.
• Preferred CPR certification or ability to obtain within 90 days.
• Strong communication, assessment, and patient education skills.
