Physical Therapist - Per Visit

About the Role

The Physical Therapist in Home Health assesses patients' functional status, mobility, and medical conditions to develop and implement personalized treatment plans. They provide interventions to improve or maintain patient well-being, monitor progress, and ensure effective communication with patients, families, and the healthcare team.

Responsibilities

• Evaluate patient needs and assist in developing therapy plans with physicians
• Deliver therapy services within legal scope and in coordination with team members
• Observe and report patient responses and condition changes
• Perform treatments to relieve pain and restore or maintain function
• Educate patients, families, and healthcare staff about care plans and progress
• Complete evaluations, develop care plans, and document progress
• Ensure care is appropriate irrespective of patient background or circumstances

Requirements

• Current state licensure in Physical Therapy
• Valid driver's license, vehicle insurance, or access to transportation

Preferred Qualifications

• CPR certification or ability to obtain within 90 days
