Physical Therapist - Per Visit

About the role

A Physical Therapist in home health assesses patients' functional status, mobility, endurance, and medical conditions to develop personalized treatment plans. The role involves providing therapeutic services within legal scope, monitoring patient responses, treating pain, restoring or maintaining function, and educating patients, families, and healthcare team members about care plans and progress.

The therapist must be adaptable to diverse home environments and committed to delivering equitable, quality care.

Responsibilities

• Visit patients in their homes to provide therapeutic services.
• Document visits and maintain accurate records of patient progress.
• Develop and update care plans in accordance with state guidelines.
• Report outcomes and adjust treatment plans as needed.
• Educate patients, families, and healthcare team members on care plans and progress.

Requirements

• Current state licensure as a Physical Therapist.
• Valid driver’s license or reliable transportation.

Qualifications

• CPR certification preferred.
