Physical Therapist, Per Diem, Homecare - Stratford/Milford areas

Duties And Responsibilities

• Provides therapeutic services in the patient’s home setting.
• The services provided by the Physical Therapist will follow a specific written plan of care established in coordination with a physician.
• Develops, implements, and evaluates the plan of care.
• Works to coordinate the plan of care, collaborating with other health professionals.
• Identifies patient care needs and makes recommendations for additional home health care services if needed.
• Instructs, motivates, and assists patients/caregivers in learning and improving activities of daily living.
• Demonstrates timely submission of paperwork and accurate computer documentation.

Requirements

• Graduate from a physical therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
• Currently licensed in the state(s) in which practicing.
• Two (2) years homecare experience, preferred.
• Ability to exercise initiative and independent judgment.
