Physical Therapist - Home Health - Per Diem

About the role

Join UVM Health’s Adult Home Care team and help patients maintain independence, improve mobility, and enhance overall well-being—all in the comfort of their own homes.

We are looking for therapists that are available weekdays, no weekends available at this time. Must have prior PT experience; home health experience preferred but not required!

Responsibilities

• Perform comprehensive evaluations and assessments
• Develop individualized treatment plans tailored to each patient’s needs
• Deliver skilled therapy interventions to improve mobility, strength, and function
• Support patients in remaining in their homes safely and independently

Requirements

• Bachelor’s degree or higher in Physical Therapy
• 2+ years of general PT experience, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
• Current Vermont Physical Therapist license, or willingness to obtain one

Qualifications

• Home health experience preferred, but not required, we can train the right person!
