About the role

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility, and independence from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you’ll empower individuals to overcome limitations and enjoy a better quality of life.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with the physician’s Plan of Treatment and the patient team’s goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family.
• Submit all required evaluations, plans, progress notes, and discharge summaries.

Required qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Preferred qualifications

• One year of experience as a Physical Therapist.

Pay

Per visit/unit rate: $59.00 – $83.00.
Pay range (full‑time, 40 hours/week): $93,000 – $128,000 per year. The actual amount may vary based on geographic location, skills, experience, education, and certifications.

Additional information

• TB Statement: This role is patient‑facing and part of Humana’s Tuberculosis (TB) screening program. Selected candidates will be required to be screened for TB.
• Driving Statement: This role is part of Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance meeting state minimum limits (or $25,000 bodily injury per person / $25,000 bodily injury per event / $10,000 property damage, whichever is higher).
