Per Diem Physical Therapist providing in-home care within a supportive and innovative team environment.
Responsibilities
- Evaluating patients’ needs and developing individualized treatment plans.
- Planning, implementing, and adjusting therapy programs to restore function and promote independence.
- Collaborating with physicians, patients, and families to establish treatment goals.
- Instructing caregivers and healthcare staff on therapy techniques.
- Providing accurate documentation and effective communication with patients and team members.
- Ensuring care delivery complies with policies and professional standards.
Qualifications
- Graduation from a Physical Therapist program.
- Licensed or eligible for licensure in New York.
- Ability to travel within designated geographic area.
- Home health experience preferred but not required; training provided for new graduates.
