Per Diem Physical Therapist - Home Care

Role Overview

As a Guaranteed Flex Pay Physical Therapist, you will provide personalized, home-based rehabilitative therapy to patients recovering from surgeries, illnesses, or accidents. You will conduct assessments, develop treatment plans, and monitor progress while educating patients and collaborating with interdisciplinary teams.

Key Responsibilities

• Practice independently within the community setting.
• Perform comprehensive patient assessments and create tailored treatment plans.
• Deliver one-on-one physical therapy services in patients' homes.
• Adjust care plans based on patient progress.
• Utilize innovative technology for efficient documentation.
• Educate patients and caregivers to promote recovery and independence.
• Coordinate with physicians and healthcare team members.

Qualifications & Skills

• Minimum of one year of physical therapy experience preferred or successful completion of a rehab intern program.
• Current New York State Physical Therapist License and Registration.
• Valid driver's license may be required depending on territory.
