Queens, NY – Personal Touch Home Care is expanding our Special Needs Program and seeking compassionate Physical Therapists who are passionate about delivering high-quality care in the home setting. This is a fee-for-service, 1099 contracted position. A valid and active New York state license is required.

Responsibilities
- Assess and evaluate the therapeutic, rehabilitative, and functional status of patients
- Participate in the development of comprehensive care plans tailored to each patient’s needs
- Evaluate the home environment to ensure it supports the patient’s rehabilitation goals
- Assess muscle strength, mobility, gait, range of motion, and the patient’s potential for rehabilitation
- Instruct patients and caregivers in the use and care of therapeutic appliances
- Maintain detailed records of patient assessments, treatments provided, progress notes, and discharge plans
Requirements
- Licensed to practice as a physical therapist within New York State
- Degree from a baccalaureate or master’s program in physical therapy approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association
- Two years of appropriate experience as a physical therapist preferred; home health experience is a plus
- Good verbal and written communication and organization skills
