Physical Therapist - Per Diem

About the role

We are seeking a skilled Per Diem Orthopedic Home Physical Therapist to evaluate and treat patients recovering from total hip and knee replacements and other orthopedic conditions in a home setting.

Responsibilities

• Educate patients on safe mobility, transfers, and ambulation
• Conduct initial evaluations and develop individualized treatment plans
• Adjust treatment plans as needed
• Perform functional training including ADLs/IADLs
• Document patient progress following regulatory and organizational guidelines
• Collaborate with interdisciplinary teams
• Ensure safe and effective interventions

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate preferred
• Licensed in the practicing state
• Completed CEU requirements
• CPR certification
• Strong communication, clinical decision-making, and computer skills
• Experience with EMR software is a plus
