Physical Therapist Per Diem

Role: PRN Physical Therapist in Home Health Responsibilities Evaluate patients' functional status, mobility, pain, and medical conditions to develop personalized treatment plans.Provide therapy services within legal and professional standards, collaborating with healthcare team members.Monitor and report patient responses and condition changes to supervising staff.Administer treatments to relieve pain, restore function, and maintain optimal health.Educate patients, families, and healthcare team members about care plans and progress.Conduct patient visits, document progress, and develop plans of care following state guidelines.Report evaluation outcomes, goals, and projected care frequency.Adapt to diverse home environments, ensuring respectful and effective care regardless of client background. Qualifications & Skills Valid Physical Therapy license in the state of practice.Driving license, reliable transportation, or access to public transit.Preferred CPR certification or ability to obtain within 90 days.
