Physical Therapist | PACE Clinic

About the role

Care Resources is seeking a part-time Physical Therapist to join our team of health care professionals. This position provides physical therapy services to participants at our multi-disciplined clinic and day center in Grand Rapids, with occasional home visits for participants in Kent County.

Responsibilities

• Performs intake screenings as assigned and makes appropriate recommendations for services.
• Completes participant evaluations as needed and develops a plan of care.
• Provides treatment and implements physical therapy procedures.
• Trains staff and caregivers on follow-through programs.
• Participates in care plan development and discharge planning for participants.
• Assesses the need for adaptive durable medical equipment, specialized shoes, and prosthesis.
• Communicates therapy progress and discharge plans to the team.
• Assists in training and provides professional supervision for PTA(s) and students.
• Completes documentation as required in a timely manner.
• Develops special programs according to participant needs.
• Provides education and training to staff, participants, and caregivers.

Requirements

• Bachelor's Degree or equivalent from an accredited physical therapy program.
• Physical Therapist license in the state of Michigan.
• Valid Michigan driver's license, auto insurance, and reliable means of transportation for home visits.
• Minimum of 1 year of experience working with a frail or elderly population (preferred).
• Ability to multi-task, work independently, and collaborate within a team setting.
• Passion for serving the elderly population.

Schedule

Monday – Friday, 10 am – 3 pm (25 hours per week). No weekends or holidays.

About Care Resources

Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) in Lowell. Our services include medical care, social services, prescription drugs, medical equipment, dental, eye and foot care, hospital care, medical transportation, home care, emergency and rehab services, and social and recreational activities at the Day Center. Eligible participants must be 55 and older, Kent County residents, able to live safely at home, and meet nursing facility level of care criteria.
