About the role
This full-time Physical Therapist position is in a long-term acute care (LTAC) setting, working with medically complex adult patients who require extended hospital-level care. Typical patients include those recovering from critical illness, multi-system failure, complex medical/surgical conditions, cardiopulmonary compromise, prolonged ventilator dependence, neurological events, or severe debility.
The role operates within a strong interdisciplinary model, collaborating closely with physicians, nurses, respiratory therapists, occupational and speech therapists, case managers, and ancillary staff to drive meaningful functional outcomes.
A typical day includes performing comprehensive evaluations, developing individualized care plans, and delivering evidence-based interventions focused on mobility, strength, endurance, balance, and functional independence. Responsibilities also include early/progressive mobility, gait/transfer training, discharge planning, and patient/family education.
You will work with patients who have a range of support needs, including lines, tubes, tracheostomies, and ventilator support, in coordination with the broader medical team. Responsibilities
- Perform comprehensive physical therapy evaluations for medically complex patients
- Develop and implement individualized plan-of-care programs
- Deliver evidence-based interventions focused on mobility, strength, endurance, balance, and functional independence
- Provide early and progressive mobility, gait training, and transfer training
- Participate in discharge planning and patient/family education
- Manage patients with lines, tubes, tracheostomies, and ventilator support
- Contribute to care conferences, interdisciplinary rounding, and goal-setting
- Support continuous patient care needs, including rotating weekends/holidays as required
Requirements
- 2 years of Physical Therapist experience in Long Term Acute Care
- Active PT license (Compact accepted)
- BLS-AHA certification
- 1 professional reference covering entire work history
