About the role
In-patient Physical therapy at San Gorgonio Memorial Hospital focuses on early mobility facilitation, emphasizing safe patient handling and quality care. It aims to reduce hospital-induced deconditioning and re-admission rates through comprehensive care including patient education, family training, discharge planning, and establishing a continuum of care.
Responsibilities
- Establish and maintain a comprehensive, individualized interdisciplinary plan of care under Physicians' orders, including evaluations, treatment planning, goal setting, follow-up assessments, and patient/family education.
- Care for patients across various areas such as Emergency Department, Intensive Care Units, Med-Surg, and OB units.
- Coordinate patient's Plan of Care with other Medical and Rehabilitation Staff; participate in case conferences, utilization review meetings, and discharge planning in collaboration with Case Managers, ensuring appropriate post-discharge care.
- Maintain high-quality, evidence-based practice in accordance with Physical Therapy departmental policies and procedures, TJC/CHQ standards, federal and state regulations, Medicare, Medi-Cal, and primary insurance standards.
- Foster a constructive and supportive work environment, understanding economic constraints and promoting professional and collegial relationships.
- Participate in process and quality improvement initiatives.
- Provide appropriate supervision and support to Physical Therapy Assistants.
- Act as a clinical educator for students and other clinical staff.
Requirements
Qualifications required include graduation from an accredited physical therapy program or foreign-trained physical therapy with State Board Licensure. Current California Board of Physical Therapy licensure is also required. BLS certification is necessary.
