About the Role

The physical therapist works under the supervision of leadership, evaluating, planning, and implementing treatment programs for patient rehabilitation as directed by a physician. Responsibilities include providing patient and family education, participating in family conferences, and ensuring all necessary documentation is completed.

Responsibilities

  • Provides physical therapy services to patients according to a written physician’s plan of care.
  • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in phases of physical therapy, including goals for the patient by participating in case conferences.
  • May train patient in the use of a prosthetic device.
  • Identifies patient and family/caregiver needs for other home health services and refers as necessary.
  • Prepares and submits clinical and progress summaries based on attainment of goals.
  • Participates in discharge planning.
  • Provides physical therapy consultation to home families/caregivers when indicated.
  • Participates in Interdisciplinary Group to define and update the comprehensive plan of care.
  • Supervises Physical Therapy Assistants according to organization policy and state regulations.
  • When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two weeks.
  • Other duties as assigned.

Requirements

  • Graduate of an accredited physical therapy program (Required).
  • 1 year of appropriate experience as a physical therapist (Required).
  • 1 year of community/home health experience (Preferred).
  • Licensed Physical Therapist in the State of Missouri (Required within 60 days).
  • Basic Life Support (BLS) certification (Required within 90 days).
  • Valid in-state driver’s license (Required upon hire).
  • Must be able to travel between various system facilities and off-site locations as needed.
  • Must successfully pass a background check with the Family Care Safety Registry and other applicable state registries.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Skills and Abilities

  • Essential Technical/Motor Skills: Ability to communicate clearly to patients; fine motor skills required.
  • Interpersonal Skills: Displays a friendly, positive attitude and communicates professionally with patients, families, physicians, and co-workers. Strong verbal and written communication skills.
  • Essential Physical Requirements: Must be able to move, kneel, crouch, stoop, reach, stand, push, pull, and lift. Must follow good body mechanics.
  • Essential Mental Abilities: Ability to assess, evaluate, treat, and educate patients. Must be adaptable to differing home environments, including explaining, forecasting, organizational skills, and time management.
  • Essential Sensory Requirements: Visual, hearing, and tactile skills.
  • Exposure to Hazards: Frequent exposure to communicable diseases, toxic substances, body fluids, road, and weather hazards.
